MAKING AN APPOINTMENT
All Patients are seen by appointment only.  If you are a new patient or if you have changes to your account, please arrive 30 minutes ahead of time to complete necessary paperwork.

REFERRALS/HOSPITALIZATION
If a provider refers you to a specialist, it is your responsibility to make sure the referral is pre-authorized.  We do provide the services of a Patient Coordinator if you have questions regarding your insurance authorization, referrals or follow-up appointments. 

PRESCRIPTIONS REFILLS
Please call your pharmacy at least two (2) days prior to needing a refill on your medication. Your pharmacist will contact our office. You may also fax your refill request to 270-885-2094. Saturday and holiday refills cannot be provided.
